WHO recommends oral DAAs for people with chronic hepatitis C; these drugs can cure over 95% of cases and generally have fewer side effects than previous interferon regimens.

  • Therefore, if a doctor considers interferon in a specific situation, the patient needs to be clearly explained the benefits, risks, drug interactions, and monitoring plan before treatment.

Current hepatitis C treatment prioritizes DAA drugs

  • For individuals diagnosed with chronic hepatitis C, WHO currently recommends oral direct-acting antiviral drugs instead of interferon-based regimens.

  • DAAs can help eliminate the virus from the body in most patients. When the viral infection is controlled or cured, inflammation and liver damage can improve, and consequently, SGPT (ALT) liver enzymes can also decrease.

  • This is an important point when updating content on how to lower liver enzymes: older drugs like interferon should not be presented as standard treatment options for hepatitis C today.

If SGPT is still high after viral treatment

  • A decrease in ALT is a positive sign but not sufficient to conclude that the liver is completely healthy. Patients still need to be assessed for the risk of fibrosis, cirrhosis, and other causes if SGPT (ALT) levels remain abnormal.

  • If ALT is still high after the virus has been controlled, the doctor may need to look for additional causes such as fatty liver, alcohol, medications, supplements, or other liver diseases.

  • Therefore, the safe principle for lowering SGPT liver enzymes is: identify the correct cause, use the correct medication, and monitor the response, instead of looking for a drug that can rapidly reduce ALT.

Note 6: Caution with herbal remedies for liver enzymes

Do not self-medicate with herbal remedies to lower liver enzymes

  • Some studies show that certain active compounds from herbs can reduce SGPT (ALT) levels in people with fatty liver, but the current evidence is still insufficient to consider them standard treatment methods.

  • Notably, supplements and herbal remedies are not always safe for the liver. AASLD notes that many herbal products and supplements can cause drug-induced liver injury (DILI) or supplement-induced liver injury.

  • Therefore, if you are looking for ways to lower SGPT liver enzymes, the priority remains to identify the cause of elevated ALT and adjust factors that have proven beneficial, such as diet, exercise, weight, and alcohol intake.

Milk thistle has some evidence but not enough for routine treatment

  • Milk thistle contains the active compound silymarin, which has been extensively studied in people with fatty liver. Some meta-analyses of randomized trials note that silymarin can reduce ALT and AST compared to control groups.

  • However, the studies vary in quality, dosage, and preparation. A meta-analysis also noted that it is unclear whether a decrease in ALT directly correlates with an actual improvement in liver tissue damage.

  • Therefore, you should not arbitrarily consider milk thistle as a drug to lower liver enzymes or self-administer doses like 200 mg, 2–3 times/day without knowing the exact active ingredient content and the specific preparation being used.

Be cautious with "liver tonic" products

  • Products containing multiple ingredients such as milk thistle, turmeric, dandelion, inositol, or various herbal combinations are often advertised for "detoxification" or "liver recovery." However, the effectiveness and safety of each product can vary greatly.

  • Notably, some supplements and herbal remedies can actually be the cause of elevated SGPT liver enzymes. AASLD notes that supplement-induced liver injury can occur with many different products, and identifying the causative agent is often not simple.

  • Therefore, if your SGPT (ALT) levels are currently high, you should not simultaneously use multiple "liver detox" products, as it will be difficult to determine which products are beneficial or cause side effects.

Do not self-administer high doses or combine multiple types

  • Herbal dosages cannot be applied uniformly to everyone. The content of active ingredients can vary between products, while the quality and accuracy of product labels are not always guaranteed.

  • Inform your doctor or pharmacist about all vitamins, herbs, and supplements you are taking, especially if you are treating high SGPT liver enzymes with prescription medications.

  • If ALT increases after starting a new product, stop self-supplementing with other products and consult your doctor to assess the possibility of drug-induced or supplement-induced liver injury.

Prioritize methods for lowering liver enzymes with better evidence-based foundations

  • If ALT elevation is linked to fatty liver, lifestyle changes such as weight loss if overweight, regular exercise, limiting alcohol, and improving diet are more important than relying on a single herbal pill.

  • If the cause is viral hepatitis, autoimmune liver disease, or drug-induced liver injury, the underlying cause needs to be treated rather than just seeking a product to reduce ALT.

  • In other words, lowering liver enzymes is not a race to find the strongest "liver tonic." The crucial thing is to find the correct cause and reduce the agents that are damaging liver cells.

  • Know when to seek medical attention

    • If you are taking herbal remedies and experience jaundice, dark urine, severe itching, abdominal pain, nausea, or unusual fatigue, seek prompt medical evaluation as these could be signs of liver damage.

    • For individuals with persistently high SGPT liver enzymes, experts recommend against self-treating with supplements. Review your list of medications and herbs and determine the cause of elevated ALT before deciding to add any new products.

    Note 7: Don't just look at SGPT levels to assess the liver

    There is no single "normal" SGPT level for everyone

    • SGPT (ALT) liver enzymes are measured in U/L, and the reference range can vary between laboratories, depending on the testing method. Therefore, it should not be assumed that everyone has the same "normal" threshold, such as 4–36 U/L.

    • AASLD also notes that the appropriate ALT threshold for assessing liver health may be lower than the reference range of some laboratories. Some AASLD documents use an ALT threshold above approximately 33 U/L in men and 25 U/L in women when assessing metabolic dysfunction-associated fatty liver disease.

    • Therefore, when interpreting SGPT (ALT) levels, prioritize the reference range printed directly on the test report and evaluate it along with other liver indicators.

    The higher the SGPT increase, the more important it is to find the cause

    • Do not use a single ALT number to conclude the severity of liver damage. AASLD emphasizes that the level of AST/ALT increase in the blood does not directly reflect the extent of liver damage.

    • However, an increase above the upper limit of normal (ULN) still helps the doctor guide the level of assessment needed:

      • ALT below 5 times ULN: usually requires consideration of common causes such as fatty liver, alcohol, hepatitis B/C, medications, supplements, autoimmune liver disease, and some metabolic conditions.

      • ALT from 5–15 times ULN: requires broader assessment, with special consideration for acute viral hepatitis A, B, and C in addition to other causes.

      • ALT above 15 times ULN: requires urgent assessment for severe causes such as acetaminophen overdose, ischemic liver injury, or other severe acute liver damage conditions.

    Do not self-diagnose based solely on SGPT

    • Elevated SGPT (ALT) levels can appear in many different conditions, from fatty liver, alcohol, viral hepatitis to drug-induced or supplement-induced liver injury.

    • Doctors typically evaluate ALT along with AST, ALP, bilirubin, and liver function indicators such as albumin, PT/INR. Depending on the case, patients may need viral hepatitis tests, metabolic tests, ultrasound, or liver fibrosis assessment.

    • Therefore, the goal of lowering liver enzymes should not merely be to find ways to bring ALT to a specific number. More importantly, it is to identify and control the cause of damage to the liver or related tissues.

    Know when to seek early medical attention

    • If SGPT liver enzymes are very high, increasing rapidly, or accompanied by jaundice, confusion, severe abdominal pain, excessive vomiting, or a rapid deterioration in health, prompt medical evaluation is necessary.

    • AASLD notes that individuals with acute liver injury accompanied by coagulopathy or hepatic encephalopathy should be referred to a liver specialist for urgent evaluation.

    Monitoring trends is more important than a single test result

    • A single high SGPT result does not fully reflect the state of liver health. Doctors may request repeat tests and monitor ALT along with other indicators to see if the condition improves, persists, or progresses.

    • If ALT decreases after dietary adjustments, discontinuing alcohol, weight loss, or treating the underlying cause, that is a positive sign. However, ALT returning to normal does not necessarily mean the liver is completely healthy.

    • The safest approach to lowering SGPT liver enzymes is: identify the increase relative to the laboratory's ULN → find the cause → treat the cause → re-monitor liver indicators. This is like looking at the "engine" rather than just trying to turn off a warning light on the dashboard.
